– I understand that whilst taking every reasonable precaution, the instructor cannot be held responsible for any injuries sustained to my child during Karate training.

– I agree to give one month written notice should I wish my child to discontinue with Karate tuition.

– I agree to pay the fee in advance (Standing Order), before the end of the preceding month (ie: Jan fee paid by Dec 26th)

– I authorise Academy for Karate Goju-Ryu to take photographs of my child in any class or event and to display these photographs on their website and on other promotional material.
